Planning your visit

Before your visit
  Please complete the forms.
  Make a note of your symptoms, the order in which they appeared and anything that makes them better or worse. Also, note the results of any home testing, such as your temperature.
  Know your family and personal medical history, which includes surgeries, serious illnesses, etc. (Family is defined as mother, father, siblings and grandparents.)
  Think about your eating and drinking habits, how well and how much you sleep or exercise, etc.

What to bring
  Your insurance card.
  Medications you are taking, including prescriptions, over-the-counter drugs, herbal products and vitamins. You can keep a list of all your medications on our emergency medical record form.
  A list of any allergies, especially concerning medicine or food.
  A list of health concerns or questions for the physician.
  Completed forms.
  If children must accompany your visit, please bring small quiet toys for their entertainment.

What to wear for your visit
  Wear comfortable clothes and shoes that can be easily removed.
  Please do not use perfumes or lotions with strong fragrances as others may have allergies or breathing problems triggered by strong odors.

While you are with the physician
  Please turn cell phones off or on vibrate to eliminate excess noise and interruption.
  Tell the physician about all the information you gathered together before your visit.
  Ask what he/she thinks the diagnosis or problem might be.
  Ask what tests, if any, are needed.
  Be sure to understand what you need to do to treat the problem, and how you can prevent it from occurring again.
  Find out if you are expected to see or call the physician again.
  Be sure you understand the possible side effects of any medicines the physician prescribes.

After your visit
  Follow your physician’s advice.
  Call the physician’s office if you don’t understand any of the instructions.
  Tell your physician if you feel worse, have other problems or side effects from any medicines.
  If you have an appointment for a return visit, please keep the appointment or give at least 48 hours notice if cancelling.
